FREQUENTLY ASKED QUESTIONS

Q1. Who is eligible? A1. Insured Person must be a Malaysian or foreigner who has a valid work permit, student permit, permanent resident status or MM2H status. Insured Age Limit - 30 days old to 65 years old (Renewable up to age 80 years old). All ages refer to the age of Insured Person's next birthday age. Unemployed person, student, housewife and Insured Person below 16 years old are only eligible up to Plan 3 without weekly benefit. Retiree and Insured Person above 65 years old are only eligible for plans without weekly benefit.

Q2. Am I covered at home, work or at leisure? A2. Yes, it protects you 24 hours a day, 365 days a year, all around the world, whether you are at work, at home, traveling or playing sports (except excluded sports).

Q3. Does this plan pay in addition to any other insurance policy that I may have? A3. Yes, it pays in addition to any other insurance policy you may have except for Medical Reimbursement.

Q4. Can I purchase PSv3 PLUS for my employees? A4. Yes, you can purchase PSv3 PLUS for your employees as a Group policy under your Company's Name. There is no limitation to the group size and the premiums is the same as an individual policy.

Q5. Can I purchase PSv3 PLUS for my whole family? A5. Yes, you can purchase PSv3 PLUS for your family members, regardless of the number of members. the policy will be issued as a Group Family policy and the premiums will be charged individually.

Q6. What is Traditional Treatment benefit? A6. This benefit reimburses post-hospitalisation Traditional Treatment expenses subject to the limit per visit and the maximum limit for any one accident as stated in the Schedule of Benefits. Traditional Treatment shall mean traditional medical practitioner who is an acupuncturist, bonesetter, chiropractor, osteopath and/or physiotherapist, excluding any traditional medical practitoner who is the claimant himself or his immediate family members, i.e. siblings, spouse, child or parent.

Q7. How will I be entitled for Renewal Bonus? A7. Upon renewal of this Policy (provided such renewal is continuous and the Policy has not been allowed to lapse in any one given year) the Principal Sum Insured shall be increased by 15% per year up to a maximum of 150% of the Principal Sum Insured.

Renewal Bonus shall only be applicable to the following benefits:

 Benefit A – Accidental Death

 Benefit B – Accidental Permanent Disablement

 Benefit C – Double Indemnity

Q8. If there are changes to my life profile, must I inform the Insurer? A8. Yes, you are required to inform the Insurer should there be any changes regarding your address, occupation and personal pursuits, which would affect your risk profile.

Q9. What telephone number to call in case of emergency? A9. Zurich have an agreement with Asia Assistance Network (M) Sdn Bhd (hereinafter known as "the Service Provider"), to provide a comprehensive referral emergency assistance programme.

Insured Person may place a reverse charge call/call collect to the Service Provider’s 24 Hours service hotline at any time seven (7) days a week for any of the services as provided (Please refer to policy contract for details of services provided).

Q10. Are there any excluded occupations? A10. Yes, as per the followings:-
  • Divers

  • Police
  • Army / Military and Law Enforcement Officers
  • Aircraft Testers
  • Pilots or Crews
  • Seamen and Sea Fishermen
  • Racing Drivers
  • Jockeys
  • Oil Rig Workers
  • Sawyers and Timber Logging Workers
  • Firemen
  • War Correspondents
  • Steeplejack
  • Stevedores (persons engaged at a dock to load and unload goods from ships)
  • Persons engaged in Demolition of Buildings
  • Persons engaged in Ambulance Services
  • Woodworking Machinists

Q11. Are there any exclusions? A11. Yes, no benefits shall be payable under the following circumstances:-
  • War, Civil War

  • Pre-existing condition
  • Suicide while sane or insane
  • Provoked Murder or Assault
  • Any kind of racing other than on foot
  • Radiation, nuclear

A12. If you wish to apply for "The Personal Sentinel v3" coverage, just instruct us by e-mail or call 012-6597262 providing the following details and we will be sending you the appropriate Application Form:-

  • Your preferred correspondence address

  • Insured/s occupations

  • Insured/s age next birthday

  • Which Plan (with or without weekly indemnity) have you chosen? Plan 1, 2, 3, 4, 5, 6, 7.

  • How would you like to make your payment? (By Credit Card or Direct Bank-in to Maybank) Payment by CASH is not recommended.
