the above Annual Premiums is subjected to an RM0.87 SST on the MCO
Fees (currently at RM14.50) and a RM10 stamp duty

APPLICANT (Company), the above Annual Premiums is subjected to a 6%
SST on the above Basic Annual Premiums and a RM10 stamp duty

PREMIUMS |
The premium rate shall
also increase if the insured person's age, at the anniversary of
the second policy year, falls under a new rating band. The renewal
premiums payable is not guaranteed and the Company reserves the
right to revise the premium rate applicable at the time of
renewal. Such changes, if any, shall be applicable to all
policyholders irrespective of their claim experience according to
the Company's risk assessment. Premiums for these benefits can
only be changed if premiums for all

issued on the same term as yours are similarly changed and 90 days
written notice is given. |
NOTE:
- Upgraded Room & Board
Co-Payment - If the Insured Person is hospitalised at a
Room & Board rate which is higher than his/her eligible
benefit, the Insured Person shall bear 20% of the other
eligible benefits, described in the Schedule of Benefits for
each and every claim.
- Total benefits payable in a year
must not exceed the Overall Annual Limit.
- Total benefits payable during
the lifetime is subject to the Lifetime Limit.
